I understand that Middlesex Borough Public Schools Board of Education policy requires that students ride the bus to and from all athletic events and a departure from this requirement will release the Middlesex Borough Public Schools from any liability with adverse results that may occur. The reason for the request must be sufficiently urgent to justify not riding the bus. I agree to release the Middlesex Board of Education and its employees from all liability with reference to this above-stated transportation.
